These "Prayer Cards", as they were popularly known, were at one time freely distributed with meals served on Alaska Airlines. They were a hallmark of one of the company's revered CEO's, Bruce Kennedy. In 2012 they were removed. These can be downloaded, printed, laminated, used as bag tags, or as desired.
If something like, "...is restriced from running scripts or ActiveX controls" you will

These tags are modified with a header and trailer on top and bottom, which can be removed if desired.
